Well Child Appointments:
We believe that your child’s health can best be maintained through regular check-ups when
well.  At theses visits, we evaluate growth and development;  check hearing, vision, and
speech;  promote good safety habits and accident prevention;  discuss anticipated changes in
your child’s development and behavior;  evaluate school readiness;  prevent illness with
immunizations;  and explore questions and concerns you may have about your child’s health,
growth, development, and behavior.  

For well child visits most insurance plans cover the following visits:
2 Week                  2  Month                4  Month
6 Month                 9  Month                12 Month
15 Month               18 Month               24 Month                
Then every year from 3 Years until college.        

Some children will be seen more frequently, others less frequently, and others at different
ages.  If you feel your child needs a schedule which differs from the one above, please discuss
it with us.

Adolescents are often seen for visits without their parents.  In this way, we can try to pay
special attention to their personal needs.  Confidentiality between the adolescent patient and
the doctor or nurse practitioner will be kept unless the adolescent requests otherwise, or if
parental involvement is mandated.  As the adolescent strives for independence, problems
between him or her and the parents often arise.  If there are concerns in this or other areas,
please discuss them with us.  At some point, we will bring you back in the room to provide a
summary without interfering with your child's privacy.

Illness Appointments:
Like all children, your child will become ill at some time.  If your child appears ill and you are
unsure what to do, please call for advice.  The best time to call is between 8:30AM and 10AM
so we can arrange an appointment at a mutually convenient time.

Missed Appointments and Cancellations:
If you find that you cannot keep a scheduled appointment, please let us know as soon as
possible.  This time has been set aside for your child and could be used by another patient.  If
you do miss an appointment, one will be rescheduled, although there may be a delay.

Phone Calls:
You are welcome to speak with any of our health care providers.  Our nurses will try to answer
your questions first.  If they are uncertain of an answer, they will have a doctor or pediatric
nurse practitioner call you back as soon as possible.   Normal call back times are noon to 5PM.

Emergencies:
Most childhood illnesses are best treated by us in the office.  For extreme emergencies, such
as unconsciousness, seizures, or bad accidents, go to the closest emergency room and identify
yourself as one of our patients.  Telephone us either before going to the emergency room or on
your way to the emergency room.

We are all on staff at Strong Memorial Hospital, Highland Hospital, Rochester General Hospital,
and Park Ridge Hospital.

After Hours:
We use an answering service to take messages when our office is closed.  A doctor or nurse
will call you back as soon as possible.  If you do not hear from a doctor within a reasonable
amount of time, please call again; some messages do not transmit to us correctly.

We may share our after hour answering service with other pediatric practices or ”triage”
services.  If your child needs to be seen and we are not available, you will be seen either in the
0n-Call pediatrician’s office or in the emergency room.  You should follow up with our office the
next business day.

Insurance and Fees:
We take most major health insurances.  Please advise us when there are changes to your
policy.  We will submit the charges to your insurance company.  A charge slip will be given to
you each time your child is seen.  This is to be returned to the receptionist.  Please pay at the
time of each visit.  You will be responsible to pay in advance any co-payments, co-insurances,
and deductibles.  You will be responsible for any charges not covered by your insurance
carrier.  If you have any questions about your bill, please let us know.

Referrals:
We will make every effort to meet your referral requests.  Your health care provider is your main
source for initiating all referrals.  Try to give us a 2-3 advance day notice for referrals.

Privacy:
We will keep on file your wishes for privacy as outlined in our HIPAA privacy statement.

We share your interest in every child, your great joys and sometimes your sorrows.  We want to
work with you and your child for continued good health, adaptation to life’s stresses, and
resiliency in our increasingly complicated world.  Your comments are important to us as we
