COVID VACCINE NOW AVAILABLE FOR ALL 6MONTHS AND OVER
In order to make the vaccination process as smooth and easy as possible please follow these steps:
Go to our Patient Portal and sign up for a Covid Vaccine Appointment. If you have difficulty doing this, call the office and our staff will gladly help you. We recommend that you do not receive any other vaccinations within two weeks of your Covid Vaccine.
Within one or two days of your appointment, you will receive a reminder portal message with our Consent Form. Please print, sign, and bring with you to your appointment. If you do not get this form or want to fill out in advance, you may find it here: Covid Consent Form. This form will acknowledge that your have reviewed that Vaccine Information Sheets for Ages 6months through Age 5, Ages 5-11 or Ages 12 and Over (click the form age to review online). IF you have done this previously, there is no need to do it again.
Before you appointment, please complete the New York State Consent and Information Form online. This will ensure that your vaccine is documented in the NYS Covid Vaccine Registry - this is not needed for your second dose or subsequent boosters
Although not required, we recommend that you utilize the CDC VSafe App to track how your child is doing after the vaccine
You will wait in your car for 15 minutes after your vaccination.
You will receive your CDC Vaccine card after your second vaccine dose, which you can schedule before you leave.
